All PIs and their advisers and chairs or directors must successfully complete the CITI Human Subjects Training Program before the IRB will review any application.
The Application:
- The Application —Document written by the investigator that is an official account of the planned project. Be sure to answer all questions thoroughly.
- Supporting Documents—Documents attached to the email when the application is submitted.
- Consent documents (Consent Document(s), Assent Document(s), , Debriefing Statements (if applicable), etc.)
- Recruiting documents (e.g. advertisements/poster/flyers, scripts, emails, social media posts, letters, etc.) anything that will be said or sent to prospective subjects.
- Study instruments (e.g. surveys, questionnaires, interview guides, tests, photographs, etc.)
- If applicable, permission letters, including MOU’s, letters of support, or other assurances of collaboration.
- If applicable, grant proposal.
NO RESEARCH MAY BEGIN BEFORE IRB APPROVAL
PIs: be sure to keep copies of all materials that you submit with your IRB Review Application.
Chairs/Heads: The signatures page of the application with original signatures should be submitted in hard copy to the IRB. You will want to keep a copy for your department.
To complete the IRB Application for Review form:
- Copy the form to your own files. (Get a new form each time you make an application. The form is revised periodically as we try to improve the process. The current form has 2/24/17 in the footer.) DO NOT CHANGE THE FORMAT, do not save as a pdf. or complete or send through Google Drive.
- Enter Information into the gray boxes and SAVE (DO NOT save to a different format)
- Answer all the questions.
- Prepare all other materials (consent, surveys, questions, etc.) and SAVE
- Be sure that you have all required elements and documents. All flyers, posters, advertisements, letters, postings on the internet or any other recruitment materials should be sent along with the research protocol for examination by the IRB and must be approved prior to their use.
- If you are a:
student researcher
- send an electronic copy of the application to your faculty research advisor, who should review your application, and make recommendations for any changes you need to make.
- once your advisor has approved your application, send an electronic copy of your application materials to the chair of the department/program. (They may prefer a printed copy)
- make any recommended changes and resubmit to the chair for approval signature..

administrator/staff researcher
- send an electronic copy of the application materials to the head of your division.
- make any recommended changes and resubmit to the head.
department/program chair or division head
- If you are the PI, appoint one of your senior faculty members/administrators to serve as your reviewer.
THEN
- All PI's, make one hard copy of the signature page of the application, collect all required signatures. The chair/head will sign, check the recommended level of review. Submit the original signatures to the IRB, Bailey House. (You may hand deliver if you want to speed things up.) If you or your FA or Chair are not currently on campus, call and make arrangements to send a scan.
- Send a copy of all application materials as email attachments to the IRB Administrator peelle@kenyon.edu All materials should be exactly what you will use with subjects both in content and format. PROOF, PROOF, PROOF before you submit. The APPLICATION FORM MUST BE IN THE ORIGINAL FORMAT. Other documents may be in .pdf format.
- PI's will be notified by email that the application has been received and if any changes/additions need to be made.
- PI's will be informed of a full board review date if necessary.
- PI's will be informed by email and official letter of IRB determinations. Student PIs and their FAs will be informed by email and the official IRB letter will be sent to the student in care of the FA through campus mail.
- If your protocol is approved by the IRB, you must use the materials as they were submitted and approved. Any changes to your protocol, instruments, or procedures must be submitted to the IRB for re-approval.
- NO research activities may be carried out until the IRB has reviewed and approved a PI's application.
